Completion of Registration Process

In order to process your registration:

  • We require you to complete the Authorization of Service Fees Deduction Form.
  • Please CLICK HERE to download the form.
  • Please print, complete and sign the form, and submit by email to galoi@cpco.on.ca or by fax to 416-483-2554.
  • Upon receipt of this form, your application will be processed and CPCO will forward a copy of the Authorization of Service Fees Deduction Form to your Payroll Department to commence fees deductions.
  • Please note that your effective date is the date all completed forms are received at CPCO. CPCO Legal Services begin with this date.
